Strengthening Homeless Families
Community & Home Supports (CHS) assist homeless and at-risk of becoming homeless families with locating permanent housing and achieving greater self-determination by (1) providing a comprehensive assessment of the individual’s needs, (2) ensuring that needed services are provided by a project-funded and/or other community-based organizations and (3) providing supportive services (intensive case management) for up to six-months to help stabilize the family and remain in permanent housing. The family’s financial status, strengths and support system are evaluated to determine the most viable housing and plan of service that will best meet the needs of the family. Barriers that impact the family’s ability to achieve permanent housing are identified and addressed by providing referrals to additional services. Families are encouraged and assisted in working towards resolving barriers before seeking placement in permanent housing.
Once identified as being housing ready, assistance is provided in locating affordable, environmentally safe permanent housing. CHS’s staff assist families with establishing a household budget, locating permanent housing, move in and establishing the household. Additional supportive services are provided for at least another six-months in an effort to stabilize the family by providing support in developing home management skills, household budgeting training and establishing community roots by linking families to schools, churches, health care, shopping, transportation and recreational activities.
To further assist families in maintaining permanent housing by increasing skills and/or income, the job developer will provide career guidance and counseling, the development of job seeking and keeping skills as well as job development and placement. A computer lab is available to further assist individuals to increase their skills and/or income.
Community & Home Supports services are funded through service agreements with Community Living Services.
