Assisting the Developmentally Disabled
Community & Home Supports provides supportive services to persons with developmental disabilities as directed by their Person-Centered Plan. The person, their circle of support and CHS staff all work together to provide support that addresses their specific needs while assisting the person in fulfilling their dream. All services are provided at the direction of the person in their own homes.
Community Living Supports are provided to person with developmental disability to facilitate each individual's independence and to help them integrate into the community. The following are some of the ways we support individuals on a one-on-one basis: personal assistance, activities of daily living, shopping, medication management, relationship building, participation in community activities, personal hygiene, use of money, socialization, leisure choices and routine household care and maintenance. These supports are provided based on each individual's goals for active participation in developing independence and participating in the community.
